Mandatory Document List for Claim Submission

As per policy terms and conditions you are requested to submit all the below mentioned documents at the time of claim submission so as to avoid any delay in your claim processing (please maintain a photocopy of the same with you always)
  • Duly filled & Signed (Policy holder ) IRDAI claim form
  • Photocopy of the current year policy copy , if available all previous year policies (not applicable for Group policies)
  • Original discharge card along-with Original hospital bill & payment receipts (pre-printed bills & receipts), Original investigation reports and prescriptions
  • KYC document i.e. Voters ID card, Aadhar card, Passport etc
  • Please note ECS compliance is mandatory for the payments - Cancelled cheque (name and bank account # printed with IFSC code)
    • In case Proposer name not available on cheque, copy of passbook is required (Bank Stamp is required).
    • Name of proposer in policy copy & cheque should match.
  • Declaration form needs to be filled for PPN Hospitals / Non PPN Hospital and submitted along with documents.
  • For all surgical claims above 1 lac, submission of OT notes are mandatory.
  • For all Ortho surgeries including facial surgeries above 50k, submission of pre & post-operative X-Ray films are mandatory.
  • If claim documents submitted via e-mail or whats app Self Declaration form is mandatory to fill and submit
